ADMISSION POLICY:
In selecting applicants for admission, the following are considered:
1. Grades received in previous academic work
2. Scores achieved on the Entrance Exam
3. Recommendations from the officials of the school last attended
4. Performance during the interviews/essay test and other credentials as required
5. Certification of good moral character
6. Pleasing personality
7. at least five feet (5’0”) for female and five feet and two inches (5’2”) for male, and physically fit (without deformities/abnormalities)
REQUIREMENTS:
A. High School Graduates (Those who have not enrolled in any subject in college)
a. Two (2) photocopies of the latest 4th year grading period (if not yet available, present the 3rd year HS card & a certification that you are an incumbent 4th year student)
b. Two (2) colored ID photos (2x2 and white background)
c. Two (2) certified photocopies of birth certificate
d. Application/Testing Fee of P600.00
e. One (1) long brown envelope
B. Transferees (Those who have taken college subjects already from another school and those who are pursuing another undergraduate degree)
a. Certified True Copy of Grades(with general weighted average) marked “For Evaluation Purposes only”
b. Two (2) colored ID photos (2x2)
c. Two (2) certified photocopies of birth certificate
d. Certification of Good Moral Character
e. Application and Testing Fee of P600.00
f. One (1) long brown envelopes
C. Foreign Students (As indicated in the birth certificate)
a. An initial interview will be conducted. Bring requirements listed in B.
b. The applicant must be able to read, write, speak and understand the English Language
c. Application and Testing fee of P1,000.00
PROCEDURE:
- Present a copy of the grade to the Admissions Center for preliminary screening. Confirmation of height will be done by the Clinic. For Transferees and Foreign students, an initial interview will be conducted.
- Proceed to the Accounting Office to get a receipt and pay the application/testing fee of P600.00 at the Cashier’s Office.
- Present the receipt of payment to the Admissions Center to secure the application kit (application form and recommendation forms)
- Submit all forms and other requirements to the Admissions Center before the entrance exam date to secure examination permit.
- Take the entrance exam as scheduled. Bring the test permit to be presented to the examiner.
- Claim the test result (or check the website).
- For successful applicants, set an appointment for interview at the CON Guidance Office (St. Benedict Hall, Ground Floor. Tel. No. 7356011 ext. 4410)
- Come for the interview in proper attire. Some applicants may undergo more than one interview as may be deemed necessary by the interviewer.
- If passed, have yourself undergo the following laboratory exams from any hospital/clinic: Complete Blood Count (CBC), Electrocardiogram (ECG), Chest X-ray, Urinalysis, Fecalysis and Drug test.
- For final recommendation/decision on your application, present the results with readings and analysis to the SBC Clinic. Submit clearance from the clinic to the CON Board of Admission/Guidance Office.
9. Get the decision on your application from the Admissions Center.
- For accepted applicants, pay reservation fee of Php 2,000.00. Submit the validated reservation form to the Admissions Center. Course accreditation follows for the transferees.
- Enrollment permit is given upon submission of:
a. Form 138/Original copy of the Report Card (freshman)
b. Transfer Credentials/Honorable Dismissal (transferee)
c. other documents required (i.e. admission contracts)
10. Come promptly during your scheduled enrollment. Attend the Orientation Program.
