Senior High School

 

 

Results are grouped according to examination date and can be viewed by clicking the date. Please refer to the Examinee Number (Example: 201800123) to check your category. Then, follow instructions based on your category.


Passed:

Come for interview on the date and time assigned to you. You should be accompanied by your parent/s or guardian. Please wear appropriate attire (e.g. shirt with collar, pants – no ripped jeans, and closed shoes) and bring the following:

  • Photocopy of the National Career Assessment Exam (NCAE) result
  • Photocopy of the Report Card for Third Quarter or the photocopy of the Final Report Card for Grade 10 (if available)
  • One (1) long brown envelope

For successful interviewees, you may reserve your slot for enrollment in the amount of P2 500. Pay the amount to the Cashier’s Office. Return the validated Reservation Form to the Admissions Center. The reservation fee is non-refundable. However, it will be credited to your tuition fee upon enrollment.

Note: 

1. The reservation fee is non-refundable, but it will be credited to your child’s tuition fee upon enrollment. Check payment will be accepted with a minimum of Php 2000 cash payment.

If your number does not appear in the lists of the test results, it means that you were not able to meet the required cut-off score for passing. You may obtain a copy of the exam result from the Admissions Center.


Freshmen

Category A

1. This indicates that you have successfully met the Department’s requirements for admission upon review of your credentials and entrance examination results. Please come for interview on the date and time assigned to you. Bring a photocopy of the Report Card with latest grades or the final Report Card on the day of interview

NOTE: Applicants for interview must come in proper attire. Some applicants may undergo more than one interview as may be deemed necessary by the interviewer.

Your Waitlisted score indicates that your application and credentials will be given further review. You may arrange your schedule of interview with the CON Guidance and Counseling Office (G/F, St. Benedict’s Bldg., Tel No. +632-735-6011 loc. 4410).
Please bring a photocopy of the Report Card with latest grades or the final Report Card on the day of interview.

2. Photocopy of TRANSCRIPT OF RECORD (TOR) indicating the degree and date of    graduation If 1 and 2 are not yet available, submit the following: 

  • Certificate of Graduation  or Certificate of Candidacy 
  • Photocopy of the receipt of your request for the document/s 

3. PSA issued BIRTH CERTIFICATE:  

  • One (1) original copy 
  • One (1) clear photocopy 

4. PHILIPPINE LAW SCHOOL ADMISSION TEST (PHILSAT) results 

  • Submit original copy for Law School and for Legal Education Board and  
  • one (1) Photocopy of Personal copy  Or PhilSAT Exemption Certificate / Conditional Enrollment Permit  

5. Two (2) original CERTIFICATE OF GOOD MORAL CHARACTER from last school attended 

  • One (1) from the Dean and 
  • One (1) from the Guidance Counselor

College of Medicine

 

 

FOR RESERVATION

To officially reserve a slot, a reservation fee of P30,000.00 must be paid any time before

JUNE 21, 2019. This amount will be credited to the school fees.

Failure to pay the reservation fee will mean relegation to the wait list.

Regardless of when the reservation fee is paid, enrollment will start on May 17-June 30, 2019 by paying the amount required as down payment or full payment for the semester’s fees.

Students must submit all the original copies of the admission requirements upon enrollment.

THE RESERVATION FEE IS NOT REFUNDABLE.

After paying, a copy of this reservation agreement must be returned to the College of Medicine. Failure to do so will mean relegation to the wait list and no reserved slot.


FOR ENROLLMENT

Accepted applicants must submit the following additional requirements:

  1. Original copies of all required documents, if not submitted previously
  2. Medical clearance certificate with the following:
    1. CBC
    2. Urinalysis
    3. Chest x-ray results and plates (PA view)
    4. Two recent 2”x 2” recent pictures w/ white background
